Terms and Conditions

By purchasing a membership, you agree to the terms below.

1. BOXED by CAPS Membership. A membership consists of one product box delivery each quarter. If you sign up for a BOXED by CAPS membership, you will be billed $40.00 immediately, as a non-refundable purchase, and that quarters box will deliver within 2 weeks. Thereafter, your credit card will be charged $40.00 up to one week in advance of the subsequent BOXED release date.

2. Cancellation Policy. There are no cancellation fees. Members may cancel at any time, subject to the time frame stated below. In order to cancel your subscription, you must do one of the following: (a) email us at capsstore@columbusplasticsurgery.com, indicate that you wish to cancel and follow the instructions that we send you in response; (b) call us at 614.246.6900, indicate that you wish to cancel and follow the instructions given.

Cancellations must be in effect prior to the billing cycle of the next quarterly BOXED release.

3. Change of CreditCard or Shipping Address. All Patients are responsible for updating their Credit Card on file along with making changes to their shipping address. Patients can do this by emailing capsstore@columbusplasticsurgery.com or calling 614.246.6900
